The Goals of The South DeKalb Center for Healthy Living are to:
-Prevent illness by performing health screenings, providing health education and encouraging life style modifications that promote wellness;
-Increase positive outcomes in patients with chronic diseases by providing regular screenings and aggressive case management;
-Effectuate a decrease in the use of local emergency departments by providing primary care services during evening and or weekend hours to community residents.
-Provide High quality comprehensive primary care and preventive health services to all residents regardless of insurance status or the ability to pay.

The Mission of The South DeKalb Center for Healthy Living is to provide quality primary and preventative healthcare services to DeKalb County Residents and surrounding areas; to address the health and social service needs of this population and to provide health education to the community at large.
