Services :

$30 contribution for initial visit

$20 contribution for subsequent visits

For a contribution eligible patients will receive:

- Medical assessments
- Referrals to specialists
- Limited laboratory testing
- Surgery
- Physical therapy
- Prescription medications (non-narcotic)

- Colon Cancer Screening
- Dental
- Dermatology
- Endocrinology
- Otolaryngology (ENT)
- Gynecology
- Hepatology
- Lipids
- Neurology
- Rheumatology
- Urology

Off-site services:

- Laboratory
- Radiology Studies
- Surgery when indicated
- Medical specialist referrals when ordered
- Hospitalization

Notes :

Prospective patients are pre-screened by clinic administrative volunteers. If the guidelines for eligibility are met the next phase required the prospective patient to bring identification and consecutive paystubs for the last 30 day period (4/2 paystubs) or their most recent tax form (if filed) to the Neighborhood Health Clinic. An in-take counselor then assesses the financial information.

Patients must be:

- 19 to 64 years of age
- A Collier County resident for the past 30 days
- Employed and provide 30 days documentation of work for a minimum of 80 hours a month. A patient’s total family income must be at or below 150% of the Federal Poverty Income Level
- Patients cannot have access to health insurance, Medicaid, Medicare, Social Security, Disability Insurance, worker’s Compensation or Unemployment Benefits.

Mission/Goal/history :

The Clinic is a 501(C)3 organization that began operating in April 1999 to provide quality healthcare to low-income working, but uninsured Collier County adults, utilizing a volunteer professional staff and private philanthropy.

Today, the Neighborhood Health Clinic continues to deliver these services in a caring and supportive environment. The Clinic’s goal is to break the cycle of poverty and give hope to future generations of patients.
